Board members at Southern Health NHS Foundation Trust have met today in Southampton to discuss safety and governance issues highlighted in a damning Care Quality Commission (CQC) report last month.

A report from chief executive Katrina Percy today says that "immediate action" has been taken at the trust's Evenlode site in Oxfordshire and Melbury Lodge in Hampshire, where serious concerns for patient safety were noted - including ligature risks and the ability of patients to climb onto and fall or jump off the roof.

The new report adds: "A structured programme is in place to ensure all of the findings of the January CQC report are being addressed and those actions have had the desired impact. Further, the programme is reviewing the 2014 inspection report to ensure that the actions we have taken have delivered the outcomes required."

Southern Health offers care for adults with learning disabilities and mental health problems across the South East and is currently winding up its services in Oxfordshire.

In July 2013 Oxford teenager Connor Sparrowhawk died in a bath while under its care, which an inquest later found was partially due to neglect.
